CDC HOP: Ashland County & Menominee County/Nation Community Health
Funded by a grant from the CDC, this project will use evidence-based strategies rooted in cultural values to address chronic health issues by increasing access to healthier foods and safer locations for physical activity.
Youth Advocates for Community Health
This curriculum is designed to help leaders empower young people to take an active role in addressing health issues in their communities. Through youth-adult partnerships, participants lead projects that help enhance the health of their communities.
Regional Community Health Team
This new initiative aims to advance community health and well-being through training, technical assistance, and capacity building for community partners.
Extension Rural Health Connection
Extension Rural Health Connection leverages Extension’s Health & Well-Being Institute expertise and community assets to establish and test the feasibility of a network of Extension-based community health workers.
Maternal & Child Health Title V Adolescent Well-Being Technical Assistance
This program supports local and Tribal health departments in improving the well-being of adolescents. It provides tailored technical assistance, leveraging data and community insights to address key issues in adolescent health, including mental health, substance use, and overall well-being.
Deliberative Inquiry
Community conversations using the deliberative inquiry model to help local & Tribal health departments solve community problems by focusing on shared values and community health.
Groundwork for Community Change Course
This 3-module online course can be utilized to foster a common language and comprehension of community health issues within workplaces, coalitions and groups of young people. It covers social determinants of health, health equity and policy systems and environmental change.
